Back to School Immunization Day

Attendees will receive free school physicals and immunizations, dental cleaning and examination, vision screening, school supplies, backpacks, and haircuts.  The Illinois Secretary of State office will offer State ID, drivers license renewals and plate stickers. 

Children must be accompanied by a parent or guardian, and records for the child's immunization history are required
